Our care capabilities

Individualised support built around assessed need

We support children and adults with a range of needs. What we can provide depends on the individual's assessed needs, location, staffing requirements and our ability to deliver the package safely.

Domiciliary care

Person-centred care that helps people stay safe and independent in their own homes.

Complex & urgent care

Individualised packages for people whose needs call for more intensive or specialised support.

Supported living

Support to develop or maintain independence at home and in the community.

Dementia support

Care focused on dignity, familiarity, safety, communication and quality of life.

Physical disabilities

Personalised support designed around each person's abilities, goals and daily life.

Mental health support

Person-centred support based on assessed needs, risk management and agreed outcomes.

Children & young people

Individualised support for children, young people and their families, including complex and urgent packages.

Learning disabilities

Support provided within the scope and conditions of the registration for the relevant location.

Our regulated activities and service specialisms vary by location. Every referral is assessed against the scope and conditions of the relevant CQC registration before a package is accepted.

Why refer to us

More than staff. A care partner.

  • Person-centred planning

We start with the individual, not the commissioned hours: needs, risks, routines, communication and desired outcomes.

  • Appropriate staffing

We confirm the skills, experience and training a package needs before we agree to provide it.

  • Safeguarding

Embedded in how we deliver care, train staff, report concerns and provide management oversight.

  • Risk management

Individual risks are assessed, reviewed and built into care planning and delivery.

  • Clear communication

With the individual, their family, commissioners and every professional involved in their care.

  • Quality monitoring

Packages are monitored and reviewed so concerns and changes are picked up early.

  • Continuity

Where possible, consistent teams, so people build familiar and trusted relationships.

  • Responsive management

Managers stay involved in each package and respond quickly when circumstances change.

Our referral process

From referral to care

We want professional referrals to be straightforward, with a clear decision on whether we can safely meet the person's needs.

  1. Referral

Send the individual's assessed needs, location, support required and proposed start date.

  2. Initial review

Our team reviews the information in detail.

    What we look at
    • Needs and risks
    • Staffing and competencies
    • Hours and location
    • Desired outcomes
    • Proposed start date
  3. Assessment

Where appropriate, we assess and speak with the individual, family and professionals involved.

  4. Package planning

If we can safely meet the needs, we plan the package and its mobilisation.

    What this covers
    • Staffing and staff matching
    • Care planning and risk management
    • Training and competency
    • Rotas and communication
    • Contingency planning
  5. Commencement

Care begins once the right arrangements are in place.

  6. Review & oversight

The package is monitored and reviewed, with communication kept open with all parties.

Mobilising new packages

Need care to start quickly?

Many referrals come from an unexpected change. When an urgent package is proposed, our team reviews it straight away and tells you honestly what can be mobilised, and when.

  • Hospital discharge
  • Breakdown of an existing package
  • Change in family circumstances
  • Increased support needs
  • Provider handover
  • Urgent safeguarding or welfare concerns

Our commitment

We will never accept a package simply because care is urgently needed. We first make sure we can safely meet the person's assessed needs, because a package that breaks down helps no one.

Quality & governance

Professional care needs strong systems behind it

Our experience of commissioned care means we understand contract compliance, accurate records, performance monitoring and reporting.

  • Safeguarding
  • Safer recruitment
  • Staff training & competency
  • Care planning
  • Risk management
  • Medication management
  • Incident reporting
  • Quality audits
  • Complaints & feedback
  • Management oversight
  • Service reviews
  • Business continuity
  • Partnership working
  • Digital care records

Better information. Better oversight.

We use digital care-management systems for care recording and service oversight, giving managers clear visibility of delivery.

With the individual's consent, and within information-governance requirements, these systems can also support communication about the person's care.
